Medical History & Release Forms

These forms are required for each participant for each trip. Please fill out and bring both forms into the Rec Room before your trip.


If you’d like your release form to apply for the whole school year, fill it in as follows:

  • Activity: All Great Escapes Activities
  • Date: Fall 2012 – Spring 2013
  • Location: All Trip Locations


Registration and Cancelation Policy

  • Registration for a trip without payment will not reserve a position on the trip roster. The final balance of the trip fee is due by Monday at 9:00pm on the week on the trip.
  • In the occurrence of insufficient meals from your cafeteria meal plan, a meal charge will  incur ($3 per breakfast; $4 per lunch; $5 per dinner). Please be sure to have enough meals remaining on your weekly meal plan on the week of the trip.
  • Notice of trip cancellations are fully refundable through the Monday before the trip departs. Cancellations received after this point are subject to the forfeiture of 100% of the total trip costs.
  • Absentees will not be refunded.
  • In the event of a trip being cancelled, a full refund will be issued.
